Social Worker, PRN
We are seeking a dedicated and empathetic hospital social worker to join our skilled team at Warren General Hospital! In this role, you will provide emotional support, counseling and resources to patients and their families during challenging times. You will work closely with healthcare providers to facilitate care plans, discharge planning and community resource connections.
This is a casual/per-diem position to work as needed.
JOB DUTIES:
Performs social history and evaluation of patient and, as relevant, of family.
Prepares social services plan.
Evaluates and treats crisis situations and disabilities resulting from emotional, social, and economic stresses of illness (i.e.: Dr. Blue emergencies, terminally ill situations, stillborn or defective birth situations, DOA situations, etc.).
Assists patients and families in adapting to the patient care plan and attends treatment team conferences and assists patients and families in obtaining health care and personal financial information.
Performs discharge planning activities, providing recommendations for follow-up care and conducts home environment evaluations, and processes Lifeline referrals and maintains program records.
Provides written documentation in patient’s medical chart
Observation and special assessment of patient and, as relevant, of family
Propose plan for providing social services
Home environment evaluation, when requested
All social therapy/rehabilitation provided
Social service summary, including recommendation for follow-up care
Assists out-of-town patient families with lodging, meal and transportation arrangements.
Arranges discharge transportation for patients.
Arranges the disposition of unclaimed bodies.
Processes suspected child abuse reports to designated agencies.
Assures that dependents and personal property of involuntarily committed patients are safeguarded and secure.
Assists hospitalized persons in becoming aware of patient rights and responsibilities and advanced directives and processes requests for organ or body donation.
Notifies MH Administrator of all involuntarily commits to the psychiatric service and their subsequent discharge status; serves as the petitioner, coordinating the schedule of hearings and the distribution of commitment papers.
Obtains patient/family consent for release and transfer of appropriate patient care information among providers of care as necessary to assure continuity of care, and conducts actual exchange of data.
Obtains patient/family consent for hospital admission.
WORK/EDUCATION REQUIREMENTS:
Bachelors in Social Work or related field required.
Master and license in Social Work preferred.
2 years of recent inpatient medical behavioral health social work experience is required.
Basic computer skills are required; advanced skills preferred.
