General Description:
Delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation. Laos directs and guides patient teaching, activities of nursing and ancillary personnel while maintaining standards of professional nursing.
Essential Job Duties:
Provides direct patient care, evaluates outcomes, consults with other specialist as required and adjusts nursing care processes as indicated to ensure optimal patient care.
Applies knowledge of illnesses, injuries, and diseases and uses available resources in assessment process.
Provides clear, concise written assessment of patient on admission and subsequently in a timely manner in EMR
Summaries assessment data and makes a nursing diagnosis and/ or identifies problems which are communicated to the physician and subsequent caregivers.
Follows through, as needed, on information given by shift report, patient or family regarding concerns.
Plans care and implements plan of care for patients as required by specific care unit utilizing available resources, consulting with patient, supervisory nursing personnel, physicians, ancillary personnel, family members etc.
Demonstrates ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques.
Participates in discharge plan using all available resources and identifies need for care at home or alternatives such as outpatient treatment, extended care facility, etc.
Performs point of care testing, including testing, documentation, and quality control with skill and accuracy.
Performs essential unit specific competencies related to clinical and technical aspects of patient care with skill and accuracy based on the unit specific policies and procedures.
Documents nursing intervention, patient response, effectiveness, complications, etc.
Intervenes with proper technique, procedure and safety precautions to meet individual needs of patient
Maintains current knowledge of medications and their correct administration based on age of the patient and his/ her clinical condition.
Follows the five medication rights and reduces the potential for medication errors
Documents nursing intervention, patient response, effectiveness, complications, etc.
Communicates information to peers and physicians
Adhere to and utilize proper PPE standards
Provides self-care/ family instructions as needed.
Performs other duties as assigned
Work/Education requirements:
Graduate from an accredited School of Professional Nursing is required
Must be currently licensed as a RN in Pennsylvania
Must complete RQI quarterly with the equivalent of BLS, ACLS and PALS
NIH Stoke Scale Certification within 3 months of hire
