Who We Are

Home Care Services at Warren General Hospital provides patients in need of long-term health care with the advanced level of assistance they need.

Home Care Services administers to temporarily or permanently home bound patients living in Warren County and its surrounding area. In order to receive this care, patients must first receive a referral from their physician.  After the physician confirms that the patient is in need of skilled service, the Home Care Services’ nurses and therapists will assist this patient according to his or her needs.  If further patient assistance is needed, the hospital’s Hospice or Dialysis services are also available to provide aid. Home Care Services operates daily from 8 a.m. to 9 p.m.

In order to provide patients with the highest quality of care, Home Care Services employs a large and well-qualified staff. Both departments feature Registered Nurses, Physical Therapists, Occupational Therapists, Certified Occupational Therapist Assistants, Speech Therapists, Certified Nursing Assistants, and Licensed Practical Nurses.  All of these employees are CPR certified and many nurses have achieved either Advanced Cardiac Life Support or Basic Cardiac Life Support certification.  These departments feature several specially trained nurses.

In addition to normal medical practices, Home Care Services regularly participate in community education programs.

Telehealth Monitoring

HHC is pleased to offer in-home telehealth monitoring. This small, state of the art monitor has the ability to monitor your blood pressure, heart rate, blood oxygen level, weight, and blood glucose readings up to four times daily. The information is then transmitted over the phone line where a skilled nurse monitors the  information on a daily basis. The monitoring allows the HHC staff to notify your physician of poor results and the potential for problems. This services is offered at no cost to you, the patient.
