Who We Are

Both the department of Home Care Services and the Transitional Care Center at Warren General Hospital provide patients in need of long-term health care with the advanced level of assistance they need.

Home Care Services administers to temporarily or permanently homebound patients living in Warren County and its surrounding area. In order to receive this care, patients must first receive a referral from their physician.  After the physician confirms that the patient is in need of skilled service, the Home Care Services’ nurses and therapists will assist this patient according to his or her needs.  If further patient assistance is needed, the hospital’s Hospice or Dialysis services are also available to provide aid. Home Care Services operates daily from 8 a.m. to 9 p.m.

For those in need of around the clock care, the TCC at WGH features a medical staff on call 24-hours a day.  Inpatients who require skilled service are typically admitted to this 16-bed facility following three days of acute care at the hospital.  The TCC, just like Home Care Services, is a fully licensed facility.  It is also Medicare certified and operates according to Pennsylvania Department of Health regulations.

In order to provide patients with the highest quality of care, both Home Care Services and TCC employ a large and well-qualified staff. Both departments feature Registered Nurses, Physical Therapists, Occupational Therapists, Certified Occupational Therapist Assistants, Speech Therapists, Certified Nursing Assistants, and Licensed Practical Nurses.  All of these employees are CPR certified and many nurses have achieved either Advanced Cardiac Life Support or Basic Cardiac Life Support certification.  These departments feature several specially trained nurses.
