WGH COVID-19 Vaccination Program
Warren General Hospital is currently completing registrations for upcoming public vaccination clinics for Warren County residents only that are age 65 or older. These clinics will be for individuals that qualify under the Pennsylvania Department of Health’s most recent definition of 1A recipients.
Our clinics are based on the availability of vaccines that are distributed by the Pennsylvania Department of Health, so we cannot give specific dates at this time. A hospital representative will contact you from the information that you provide to schedule a shot when the hospital receives additional vaccines.
At this time there is no walk in capabilities.
Please understand that there are two doses of the vaccine that must be given. At the time of your first dose given, a second visit will be scheduled for approximately one month later.
When you arrive for your first dose, you will need to bring picture identification with proof of your age or a valid Pennsylvania driver’s license. Also listed to the right are two forms, a COVID Consent form and CDC Checklist form regarding your current health status. These must be completed and brought with you to your first vaccination.
