Transitional Care

The goal of the Transitional Care Center is to help patients establish a routine of self-care and achieve independence as soon as possible. Regardless of race, color, sex, age, religion or handicap, our objectives remain clear: the enhance the quality of life for each resident; to assure that the resident’s rights are honored; to maximize each resident’s medical and physical rehabilitation; to provide high quality nursing and restorative therapy care.

The 16-bed skilled nursing facility is conveniently located on the second floor of Warren General Hospital. It is designed to meet the need in our community for a program that allows hospital patients who require skilled nursing care to continue their recovery in a more home-like environment.

Our Center provides a bridge between the highly sophisticated acute care hospital and the home setting. Our goal is to help you become as independent as possible by providing the appropriate and necessary treatments, therapies and support you need to enhance the outcome of your stay in our Transitional Care Center. WE offer rehabilitative and nursing care by or under the direction of licensed professionals.

Our Center is certified by the United States Department of Health and Human Services for participation in the Medicare program, and licensed by the Pennsylvania Department of Health, Division of Long Term Care.

We are happy to be able to serve you and, while your recuperate, we will do everything we can to make your stay with us as comfortable as possible.
