Who We Are

Founded in 1979, Hospice of Warren County is a non-profit, Medicare certified, organization designed to provide excellent comfort to any terminally-ill patient who:

  • Resides in Warren County or the surrounding area.
  • Has approval of his or her physician.
  • Has a life expectancy of 6 months or less if the illness runs its normal course.
  • Has a family member or other appropriate person available to assist with continuing care needs and decision-making processes.
  • Has changed the medical goal from curative to comfort treatment.
  • Has knowledge of prognosis and has consented to care.
  • In order to provide such individuals with this level of comfort, Hospice works with each patient, and patient doctor, to design an individualized program meant to ensure the comfort of the patient, and the comfort of the patient’s family, during their remaining months.

    Throughout this individualized program, Hospice employs a team effort.  Physicians (led by the program’s Medical Director Dr. Price), certified registered nurses, social workers, clergy, a nutritionist, bereavement counselors, and 75 specially trained Hospice volunteers all work to keep patients, and their families, comfortable.  Speech, occupational, and physical therapists are also available for assistance.

  • Physician Directed Interdisciplinary Team Care
  • Regularly scheduled nursing visits
  • Registered Nurse on call 24 hours, 7 days a week
  • Home Health Aides
  • Social work services, including assistance with financial matters and social/family concerns
  • Spiritual and emotional support
  • Supportive services provided by specially trained volunteers
  • Assistance in home, nursing home, assisted care facility, and hospital settings
  • Family counseling and training of family members in patient care
  • Dietary counseling as appropriate
  • Speech, physical, and occupational therapies as appropriate
  • Medical equipment and supplies
  • Coordination with other community services
  • Bereavement counseling and support groups
  • In addition to providing its services regardless of a patient’s race, age, sex, ethnic background, or handicap, Hospice of Warren County also does not discriminate against patients based on their ability to pay.  While the program does accept payments from Medicare, Medicaid, and private insurances, community support through United Fund donations, memorial gifts, donations, bequests, and its membership drive help defray the cost of the extensive Hospice services that are not reimbursed.  The success of the program depends on this invaluable community support.  Hospice of Warren County also receives support from groups, such as the Pennsylvania Hospice Network and the National Hospice and Palliative Care Organization.
