HEALTH BENEFITS

Medical Coverage – Highmark Select Blue available to eligible full time and part time employees. Employee contributions to premium can be made on a pre-taxed basis. Available first of the month after probation.

Home Host Program – Higher level of benefits and lower employee out of pocket costs when services are performed at Warren General Hospital.

Prescription Plan – Included with medical coverage. Pharmacies include most local store and major pharmacy chains across the United States. Also, a mail order option (pay for two months and get the third month free).

Dental Insurance – Eligible employees have coverage through Benefit Coordinators. Employee contributions to premium can be made on a pre-taxed basis.

Vision Plan – Available to eligible full time and part time employees. Life Insurance – Eligible full time employees have coverage equivalent to an employee’s annual wages (capped at $50,000). Eligible part time employees have $4,000 of coverage. No cost to employee.

Long Term Disability – After a required waiting period, eligible full time employees receive 50% of gross monthly income until released by physician. No cost to employee.

RETIREMENT PLANS

403(b) Pre-Taxed Qualified Retirement Plan -
  • Employee can contribute pre-tax money to the 403(b) plan. A large choice of mutual fund options, Internet access for viewing account, and changing allocation or amounts withheld. Loans are also available with certain limitations.
  • Employer Match – The Hospital provides matching up to a maximum 4.33% of contribution. The employer matched money is generally immediately vested.
  • 4.5% Non-elective Employer Contribution – The Hospital contributes 4.5% of your biweekly pay into your 403(b) account. The Hospital’s non-elective contribution is immediately vested.

Tuition Reimbursement – Available to eligible full time and part time employees after one year of service who enroll in pre-approved job related course work at an accredited institution. The Hospital will pay up to defined limits for tuition and books each fiscal year.

Supplemental Insurance – Employees have the option to purchase pre tax additional plans. Warren General Hospital offers these programs through AFLAC. Some of the options are: dental, vision, personal disability, additional life insurance, care assist, cancer and accident plans. Only the life insurance requires the use of after-tax dollars.

Federal Credit Union – The Hospital is affiliated with National Fuel Gas (NFG#2). Payroll deductions are available.

Direct Deposit – Convenient and available to all employees to most banks.

Vacation – Based on years of service for full time and part time employees.

Short-Term Sick Time – 8 hours for part time after one year of service and 24 hours upon hire for eligible full time employees.

Long-Term Sick Time - Full time employees earn a fixed amount of hours each pay and accumulate to a maximum (certain limits apply).

Floating Holidays – After one year of service eligible full and part time employees accumulate up to 40 hours.

Paid Birthday Holiday – For eligible full time employees.

Service Awards – Recognition dinner and choice of awards at 5-year service levels.
